Abstract
The questionnaire method is often used to get review, information or opinion• about certain topic. This project is named as Questionnaire Designer or "Pembangunan ' Perisian Kaji Selidik". The objective of this project affects two main issues; producing a questionnaire and managing the data input from respondents. The results from the system developed will enable user design a questionnaire easily, manage the input data from respondents efficiently and at the same time minimize the user errors. The questionnaires given to the respondents refer to the output results from the system. The methodology used to develop the system is Prototype Model where allow the phase repeating and UML (Unified Manipulation Language) technique for the design. This system expected to help end user to analyze their questionnaire faster than before.
